How to get your Gift Card?

To qualify for the gift card from our referral program you must have one of the following:

Termite- Service Completed and Paid in FulL
Initial Service for Pest Control and 1 Quarterly-Both Services Paid in full
Initial Service for Pest Control and 2 Monthly Services* * (All three services need to be paid in full)
One-time service (Could be any service from us but the price must be $150 or greater. Also the service needs to be paid in full)

Please note: Customers will be able to make payments for services however gift cards will not be sent out until all services are paid in full.

*There are two forms below, One for existing customers that are looking to participate in our referral program and another form for new customers looking to participate as well.
